Primary Care Clinics Closure Notification

Questcare Medical Clinics is closing on July 31, 2026, with July 17, 2026, being the last day for patient care. The closure includes all our primary care clinics in the area (Arlington, Burleson, Coppell, Dallas, Flower Mound, Fort Worth, Keller, Mansfield, McKinney and Melissa).

We understand that changes in your healthcare can feel disruptive. The team at Questcare is committed to supporting you and making this transition as smooth as possible.

To request a copy of your medical record or to request a transfer of your record to a different clinician, please complete the PATIENT REQUEST FOR RELEASE OF MEDICAL INFORMATION form and email it to QCPCMedicalRecords@EnvisionHealth.com.
